DOST Scholarship Forms PDF 2026: Form A, E2, E5, I & How to Fill Up
Quick answer: DOST scholarship applicants upload Forms C to J: good moral (C), good health (D), principal’s or ALS certification (E1–E4), residency (F), parent’s certification (G), exam history (H), no post-secondary units (I) and signed declaration (J). Download the latest forms only from the official DOST-SEI portal.
The DOST scholarship forms are the certifications and declarations you upload with your DOST-SEI undergraduate application. Each one proves a different requirement, from good moral character to your strand and residency, and a missing or unsigned form can stop your application at validation.
This guide lists every DOST scholarship form PDF for the 2026–2027 cycle (AY 2027–2028), explains what Form A, the E-series forms, Form I and the guardianship affidavit are for, and shows how to fill up the DOST scholarship form step by step.
Important: always download the latest forms from the official portal. Forms from previous years may have different names or content.

Application Forms (Form A / B etc.)
Older DOST scholarship application forms came as paper forms. Today, the information in Form A and Form B is filled in directly in the online E-Application System.
- Form A – Personal Information: in the old paper version, this was the main DOST scholarship application form. It asked for your name, address, school, family details and the program you were applying for, with a photo attached on the first page.
- Form B – Household Information Questionnaire (HIQ): required only for RA 7687 applicants. It measures your family’s socio-economic status.
Prepare this information before you start the online HIQ:
- Father’s and mother’s annual gross income
- Guardian’s or spouse’s income, if they support you
- Any outside financial contribution: who gives it, how much and for what
- Hectares of agricultural land the family owns, if any
- Type, year and model of any family vehicle
Form E2, E3, E4, E5 – What They Are
The E-series forms are the principal’s or coordinator’s certification of your academic track. Submit only the one that matches your situation:
| Form | Use it if you are… |
|---|---|
| E1 | A graduating or graduated Grade 12 student in the STEM strand |
| E2 | A Grade 12 student in a non-STEM strand who ranks in the top 5% of the class |
| E3 | A homeschooled applicant |
| E4 | An Alternative Learning System (ALS) learner |
| E5 | A high school graduate from before the K-12 program in the top 5% of the graduating class (used in earlier cycles; check if it appears in the current portal) |
Form I and Other Scholar Forms
- Form I: your certification that you have no post-secondary (college or vocational) units. Earning any college units makes you ineligible for the undergraduate scholarship.
- Form G (undergraduate): your parent certifies that you have no pending immigration application to another country.
- Form G1 / G2 (JLSS): commitment to return service, G1 for Merit or RA 7687 applicants and G2 for RA 10612 applicants.
- Form H: your history with the DOST-SEI exam, for example if you took it before and did not qualify.
- Form J: the signed declaration of both the applicant and parent or legal guardian that all information is true.
Once you become a scholar, other forms come into play, such as the Amendatory Agreement for shifting or transferring and the clearance forms explained in our DOST scholarship handbook guide.
Affidavit / Guardianship Forms
If a legal guardian, not your parents, supports you, you must upload a notarized Affidavit of Guardianship. It usually includes:
- The guardian’s full name, address, age and relationship to you
- A statement that the guardian supports your schooling and daily needs
- The reason your parents cannot support you (deceased, working abroad, separated, etc.)
- The guardian’s signature, a valid ID and the notary’s seal
